AutoUpgrade Tool Fails With Error Unable to determine DB unique name

SYMPTOMS
When initiating upgrade via Autoupgrade tool, it reports below error.

<ORACLE_HOME>/jdk/bin/java -jar <ORACLE_HOME>/rdbms/admin/autoupgrade.jar -config <config_file> -mode analyze
AutoUpgrade tool launched with default options
Processing config file ...
Unable to determine DB unique name for entry <DB_SID>


CAUSE
Autoupgrade tool couldn't identify the database name

SOLUTION
Bounce the database once and then re-initiate Autoupgrade
