CAUSE
Issue is related to Bug 30621255  - QUERY FAILS WITH ORA-07445: [QKACCO()+91] as of 30th April 2020 the Bug was still been Worked upon by Development team



Version Affected 19c

SOLUTION
Error is reproducible even when run from sqlplus



Set first at session level and see if the workaround assist

alter session set "_optimizer_nlj_hj_adaptive_join"=FALSE;

If it works set it at system level .

Its Normally used for adaptive joins that switch from nested loops to hash joins

So Impact may very you would need to compare the awr report after setting this parameter
